We will always be understanding that appointments may be missed for a variety of reasons. We request notice of cancellation 24hours prior to the visit. We understand that sometimes situations arise and 24hours will not be possible. However missing appointments without notice or adequate reason will not be tolerated and will be subject to the following missed appointment fees:

• Missed Appointment, Consultation, Annual Physical without adequate notice (24 hours) - $100.00

• Three or more missed appointments without adequate notice may result in dismissal from the practice. Failure to pay for missed appointments will result in dismissal from the practice

• We will not charge missed appointment fees without 24 hour notice if family emergency, traffic or adequate explanation.

• Missed appointment fees will include failure to attend virtual video/teleconference appointments without notice.
